Medical Management

Offering complete women’s gynecological care including preventative services, contraceptive care, gynecological assessment and treatment for issues related to the female reproductive system.

Management of mean’s health to assess, educate on possible risk factors for and treat various conditions.

If End Stage Renal Disease (ESRD) is diagnosed in any patient, the ESRD stage is confirmed and patient is educated on the disease and monitored in the community to slow down the progression of the disease and education for life style changes is provided.

Patients with stage IV ESRD are educated regarding dialysis options, access site is created for when patient becomes at stage V and requires dialysis.

Medical Management: Renal diet education is provided to patients who have ESRD and do not yet require dialysis and also to dialysis patients.

Medical Management: Hemodialysis is provided to patients with Stage V ESRD, medical team managed the patients

Patient Evaluation and Treatment

We evaluate and treat persons who may develop symptoms of Hypertension (HTN) and/or Diabetes (DM) and need to be placed on a medical regime.

In addition, we screen persons who wish to have their renal function evaluated with the aim of ruling out Chronic Kidney Disease (CKD)/End stage renal disease (ESRD).

We provide a range of primary health care services that include but are not limited to family planning and sexual health services. In addition to providing preventative care to our patients, our aspiration is to become a champion of women’s and men’s sexual health at every life stage and can deliver the best possible advice and care to women and men.

When called to the hospital by the medical team, evaluation is done for patients who are in hospital due to Kidney Failure and requiring hemodialysis.

We evaluate and treat patients with renal related issues such as Glomerulonephritis, polycystic kidney disease, Pyelonephritis, Nephrotic syndrome, Renal calculus etc

Health Solutions Inc. Hemodialysis is a way to remove waste products and excess fluid from the blood when the kidneys are no longer able to do their job adequately. In hemodialysis, blood is removed from the body and filtered through a filter called a dialyzer (artificial kidney) and the filtered blood is returned to the body.

One important step before beginning hemodialysis is preparing a vascular access, a site on your body from which your blood is removed and returned. It will allow for easier and more efficient removal and replacement of your blood with fewer complications. There are three main types of access points for hemodialysis: arteriovenous (AV) fistula, AV graft and central venous catheter.

During the typical dialysis session, the patient will need to sit or lie comfortably in a chair or bed, where he/she can read, listen to music, watch TV or sleep. Patients are also welcome to use their mobile phone to make any calls, work on a PC, or socializing with other patients. (Free Wi-Fi is provided).

Health Solutions Inc. uses High-flux dialyzers for hemodialysis which allows removal of both small and middle molecular weight solutes and in turn improves the results of dialysis for the patient.

Once the patient is settled in, using sterile technique the patient is connected to the machine to begin dialysis. The lines and the dialyzer are connected to the dialysis machine. The machine safely controls the flow of blood through the blood pump. Each patient based on their prescribed renal regime will have specific settings placed into the machine to ensure appropriate dialysis clearances and adequate fluid removal is achieved. A complete dialysis session usually lasts between 3.5 – 4 hours. The ideal dialysis recommendation regime is dialysis minimum 3 times a week with 3.5 – 4 hour sessions each.

Nurses periodically monitor both the patient and the dialysis machine to ensure patient stability.

Hemodialysis itself is not painful. However, it is possible for the patient to experience some side effects. It is quite common to feel tired after a long session of dialysis. Some patients may have nausea, feel dizzy or have muscle cramps during dialysis. If you have any of these problems, the medical staff will do everything possible to help you cope and re-evaluate your prescribed regime to avoid it from happening again.

Through a surgical procedure, an AV fistula connects your artery directly to a vein, typically in the forearm. Connecting the artery to the vein causes more blood to flow into the vein. As a result, the vein grows larger and stronger, making repeated needle insertions for hemodialysis treatments easier. This method is often ideal because your own tissue is used, decreasing the chance of infection and clotting. Keep in mind, when a fistula access is created you will need to give it time to “mature” for use approximately 8 – 10 weeks. To develop your new fistula, be sure to exercise your access arm by squeezing a foam ball or using hand grips as directed. This helps the fistula develop properly.

Another option is to implant a synthetic tube, or graft, to connect your artery and vein under the skin in your arm. The graft becomes an artificial vein that can be used repeatedly for needle placement and blood access during hemodialysis. A well-cared-for graft can last several years and maturation is not required.

  • Thoroughly clean your fistula before each dialysis session.
  • Touch your fistula every day to feel the rhythm of your blood flow called “thrill” (vibration). The “thrill” tells you that the blood is flowing through your access as it should be. If you do not feel the “thrill,” you should call your dialysis unit as this could signal a clot.
  • Your access site is only to be used for dialysis treatments.
  • Do not allow blood to be drawn from the access.
  • Do not let anyone put a blood pressure cuff on your access arm.
  • Be careful not to bump or cut your access.
  • Do not scratch your access site as the scratching can increase the risk for infection if the surface of the skin is broken.
  • Do not wear jewellery or tight clothes over your access site.
  • Do not sleep with your access arm under your head or body.
  • Do not lift heavy objects, carry children, or put pressure on your access arm
  • Report any areas of your graft or fistula that appear to be protruding or ballooning out. This is called an aneurysm. The skin over an aneurysm may look thin and shiny.
  • Check your access point every day for signs of infection. These can include swelling, tenderness, or redness. If you experience any of these symptoms, contact your doctor.

If your kidney disease has progressed quickly, you may not have time to get a permanent vascular access before hemodialysis has to be initiated. A decision is then made to create a tunnelled catheter access site. The catheter is a long Y-shaped tube which the tip can be placed in your neck, chest or groin and is connected to a central vein. The other end of the tubing exits your skin and is attached to the tubing on the dialysis machine. Once the catheter is inserted it can be used immediately for dialysis. This choice of access is only used as a last resort as most catheters have a high infection rate and clot more frequently.

  • Do keep the catheter area clean and dry
  • Do not pinch, poke, bend or pull the catheter
  • Do not use sharp objects, such as scissors around the catheter
  • Do not open your catheter. The end caps of your catheter MUST be kept tightly closed. Only your dialysis nurse or physician should remove the caps or clamps.
  • Do not get the catheter wet in the shower. Avoid getting the waterproof dressing wet.
  • Ensure you do not go in the sea or a swimming pool
  • Contact the unit if you develop a high temperature, chills, shivering or sweating.
  • Contact the unit if you have any pain, redness or swelling around the exit site of the line.
  • Contact the unit if you notice any bleeding from the catheter site.
  • Any medical staff who touches the catheter site must undertake strict procedures to prevent infection. Ensure the dressing is not touched by anyone other than medical personnel who are familiar with using sterile technique.

In an ultrasound, sonographic sound waves outline the shape of various tissues and organs in the body to produce images. These images assist the radiologist and/or medical provider in diagnosis and treatment of conditions.[/vc_column_text][mk_toggle style=”fancy” title=”What Happens During The Test?”]

Our technologist will take a brief medical history. You will be asked to lie down on the scanning table. The ultrasound technician (sonographer) will place a clear gel on the area of the body to be imaged. The sonographer will then press the transducer against the skin and sweep it back and forth over the area of interest. The transducer is a small hand-held device that resembles a microphone, attached to an ultrasound machine by a cord.

After the exam, the gel is wiped off, and you may resume normal activities.

Ultrasounds typically take up to 30 minutes, depending on the exam performed.

Health Solutions believes that screening is critical to the detection of breast cancer at any stage. Annual screening mammography is currently recommended for all women over the age of 40. Our equipment can detect a cancer in the breast in some cases up to two years before a patient or physician can feel the lump. Make Health Solutions your screening centre as we employ the best staff in digital mammography and use state of the art equipment to perform mammograms for all patients.

We have found that following screening mammography, about 10% of women will be recalled for additional views. This should not cause undue worry as more than 9 out of 10 patients that get recalled for additional views are subsequently categorized as “benign” or “probably benign”.

If it is determined that additional views are necessary, one of our staff will contact you and schedule an appointment. If a biopsy is deemed necessary, we will schedule an appointment and make all arrangements to have this procedure completed, pending authorization from you and/or your doctor.

Mammography is a simple, quick, non-invasive screening test used in the prevention and early detection of breast cancer. The procedure uses low-dose digital imaging to safely create images of the breast tissue. Radiologists read the images to determine whether cancer is present. During the procedure, your breast is placed on a platform in the mammography machine and compressed between two plates (to spread all of the breast tissue out so nothing can “hide” and to have view of all breast tissue area).

You may want to dress comfortably and wear a 2-piece outfit because you will need to undress above the waist. You should not wear any type of powders, deodorants, ointments, or creams when you come for your mammograms because it can affect the quality of your pictures. You may want to avoid scheduling your mammogram for the week before your period if your breasts are usually tender during this time. The best time to have your mammogram done is one week following your period.

Important things to tell your technologist:

• If you have a personal history of breast cancer
• If you have a family history of breast cancer
• If you have any lumps, pain, nipple inversion/discharge, or dimpling of the breast
• Where and when your last mammogram was done
• If you are pregnant
• If you have breast implants

Note: The technologist staff is trained to acquire images for a radiologist to review. The radiologist is specially trained to look at x-ray studies and make a diagnosis off of them. The technology staff is not fully trained to do so, therefore we will not be able to give you results at the time of your exam.

At Health Solutions, bone density testing is available by appointment. This is part of our approach to providing preventative care to our patients as Bone Density provides diagnosis of osteoporosis and assesses an individual’s risk for developing fractures. It is quick, non-invasive, and uses less than 1/10th the dose of a standard chest X-ray. Osteoporosis is a debilitating bone loss condition that affects one in three women after menopause. Although osteoporosis typically afflicts women, men can also develop bone loss. For women, a baseline scan is usually recommended at age 50 or following menopause. Subsequent scans are typically ordered every 2 years.

This test is one of the steps in forming the diagnosis and overall management of osteoporosis. While 1 in 8 women have a lifetime risk of developing breast cancer, 1 in 4 women have a lifetime risk of developing osteoporosis (1 in 8 for men).

Osteoporosis is also silent and only becomes apparent when a bone is fractured from what would generally be considered a minor fall (fragility fracture). The bones that are commonly involved include the wrist, hip and spine.

Bone density is measured using a process called a dual-energy x-ray absorptiometry, or what’s commonly known as a DEXA scan. This procedure measures the density of bones in areas of the body that are prone to breaks and fractures, such as the spine, hips and forearms.

Do not take calcium supplements for at least 24 hours before the exam. Dress comfortably, avoiding garments that have zippers, belts or buttons made of metal. You may be asked to wear a gown.

Age—as we grow older, our risk for breast cancer increases.

What should I know before getting a mammogram?

Women have different types of breast tissue. Breasts are made up of a mixture of fibrous and glandular tissue and fatty tissue. Your breasts are considered dense if you have a lot of fibrous or glandular tissue, but not much fat. Having dense breast tissue may increase your risk of getting breast cancer.

Why is breast density important?

Dense breasts make it more difficult for doctors to spot cancer on mammograms. Dense tissue appears white on a mammogram. Lumps, both benign and cancerous, also appear white. So, mammograms can be less accurate in women with dense breasts.

How do I know if I have dense breasts?

Breast density is determined by the radiologist who reads your mammogram. There are four categories of mammographic density. The radiologist assigns each mammogram to one of the categories. Your doctor should be able to tell you whether you have dense breasts based on where you fall on the density scale.

Density scale:
Almost entirely fatty
Scattered areas of fibro glandular density
Heterogeneously dense
Extremely dense

If I have dense breasts, do I still need a mammogram? Yes. A mammogram is the only medical imaging screening test proven to reduce breast cancer deaths. Many cancers are seen on mammograms even if you have dense breast

Breast ultrasound is used to help diagnose breast abnormalities seen on mammography or felt during an examination. It is typically ordered following a mammogram or to assess a lump or focal pain.
Thyroid ultrasound is often performed for an enlarged thyroid, abnormal thyroid enzymes, or an abnormality seen on another imaging modality.

Osteoporosis

Osteoporosis is a disease that is characterized by a decrease in bone mass and density. The decrease in bone mass leads to an increase in the risk of fractures.
Osteoporosis is a debilitating bone loss condition that affects one in three women after menopause.
Although osteoporosis typically affects women, men can also develop bone loss. For women, a baseline scan is usually recommended at age 50 or following menopause. Preceding scans are typically ordered every 2 years.

Even though osteoporosis affects different ages and ethnicity, there are a few factors that make people more susceptible to developing the bone disease.

Am I at Risk for Osteoporosis?

Your risk for osteoporosis is higher if you:

• Are post-menopausal woman who is not taking estrogen
• Are post-menopausal woman who is tall (over 5’7″) or thin (less than 125 pounds)
• Have a personal or maternal risk of hip fractures
• Are a man with clinical conditions associated with bone loss.
• Use of medications known to cause bone loss, such as corticosteroids, Dilantin, or high dose thyroid replacement drugs
• Have type 1 diabetes, kidney or liver disease, or family history of osteoporosis
• Suffer from hyperthyroidism or hyperparathyroidism
• Have had X-ray evidence of vertebral fracture or other sign of osteoporosis
• Have experienced a fracture after a mild trauma

Osteoporosis Treatment Options

The main goal of treatment will be to prevent fractures and breaks. In addition to recommending a proper diet rich in calcium, the doctor may prescribe medication. The following is not a list of all medication options, but simply the most commonly used:

• Bisphosphonates. This type of medication slows cell activity that is responsible for bone loss. Bisphosphonates are intended to maintain or even increase bone density.
• Parathyroid Hormone. This option is for postmenopausal women who are at high risk for fracture.
• Estrogen Agonists/Antagonists. Typically used to treat postmenopausal women, these medications are not estrogen, but they have estrogen-like effects on the body.
• Calcitonin. Meant for women who are at least five years into menopause, calcitonin helps to regulate calcium and bone metabolism.

Osteoporosis is a condition that shouldn’t be taken lightly. A broken bone can result in hospitalization and even surgery.

What is a Breast Biopsy?

A breast biopsy is used when an abnormality that has been detected by ultrasound, mammography, or clinical exam is found, and tissue acquisition is necessary to determine its nature

At Health Solutions Well Woman Clinic, we offering ultrasound guided biopsies. The procedure is performed in a convenient, comfortable outpatient setting. The procedure is minimally invasive, non-scarring, and non-surgical. You can return to normal activities the following day.

What Happens During The Test?

Our clinic nurse will take a brief medical history. You will be asked to change into a gown and lie down on the scanning table. The doctor will apply a local anesthetic to your breast and insert a needle into the breast abnormality, using imaging guidance technology. Samples are obtained, and a small surgical marking clip is placed in the area biopsied, which will identify the site for future reference. The marker will cause no pain, disfigurement, or harm. Following the procedure, your skin will be covered with Steri-Strips to protect the biopsy site and promote healing. A follow-up mammogram is then obtained.

Plan for a 2-hour appointment, which includes a radiologist/physician consultation and possible additional mammogram images following the procedure. The procedure itself will take 30 to 40 minutes to complete.

How Do I Prepare for the Test?

Prior to the exam, do not take any products containing aspirin for at least 3 days prior to the exam. If you are on blood thinners, you will be instructed to discontinue these medications. Wear a comfortable two-piece outfit. You will be asked to change into a gown. Eat a light meal. Plan to be off work the day of the procedure. Most women can return to normal activities the following day. You will not need a driver.

When Can I Expect the Results?

Samples are sent to lab for analysis. We will contact you within 5 business days with the results. Copies of your results can be provided to you or faxed/e-mailed to your primary care physician.

After the test

You will be sent home with an ice pack, Steri-Strips, and post-procedure instructions. The site will be tender for a few days, and bruising will likely be present. Most women experience a moderate ache, which is usually controlled by an ice compress and acetaminophen. Avoid strenuous activity, and get plenty of rest. You may bathe the morning after the biopsy, being careful not to soak or scrub the biopsy site.

Its capital is Kingstown, also its main port. The country has a French and British colonial history and is now part of the Organisation of Eastern Caribbean States, CARICOM, the Commonwealth of Nations, the Bolivarian Alliance for the Americas and the Community of Latin American and Caribbean States (CELAC).

Agriculture, dominated by banana production, is the most important sector of this lower-middle-income economy. The services sector, based mostly on a growing tourist industry, is also important.
